We want your visit to be as convenient as possible. The information below will help you plan your visit to Prestige Urgent Care.
Participating Insurance
Prestige Urgent Care of Shreveport participates in most insurance plans. Please contact member services (phone number is generally located on the back of your insurance card) to verify participation or call our facility so that we can check your eligibility for you. You are responsible for your in-network Prestige Urgent Care or Professional Office Visit benefits. All co-payments are due at the time of service.
Non-Participating Insurance
If you have health insurance, but we do not participate with your plan at this time, you are still welcome to be seen. However, you will be responsible to pay for your visit at the time of service.
We offer a low cost rate that is competitive to other urgent care facilities throughout the state of Louisiana.
No Insurance (Self-Pay)
Prestige Urgent Care offers a self-pay option starting at $150.00. Any additional services such as x-rays, sutures, labs, injections, etc, are collected in addition to the office visit. Acceptable methods of payment are Cash or Credit/Debit Card. All self pay accounts must be paid at the time of service.
Forgot Your Insurance Card?
Our office staff will attempt to verify a patient’s insurance eligibility by accessing the insurance company’s website. If insurance eligibility can’t be verified, or a patient is unable to provide their insurance identification card, the patient must pay for the visit in full at the time of service. Self-Pay rates will apply.
Payment Policy
Payment for services rendered are collected at the time of service. This includes co-payments.
We accept Cash or Credit/Debit Card payments.
All past due patient balances must be paid in full before being seen.
We are pleased to be able to serve the medical needs of the Shreveport – Bossier City community. So that we can continue to provide the high level of service that you have come expect, and that you deserve, we ask that patients pay their bills promptly and fully.
No Fault Insurance MVA (Motor Vehicle Accident)
No-Fault law was enacted in the 1970’s and applies to anyone injured as a result of an automobile accident or any action taken to get in or operate a car. This includes the following:

Fastening seat belts

Slamming fingers in car door

Falling getting into or out of a car
-
Loading a car or truck
Per Louisiana No-Fault law, we must bill your insurance company for the medical services rendered. We cannot submit these claims to your health insurance company.
The following information MUST be provided:

Date of accident/injury

The auto insurance carrier’s name, address (not the Insurance Agent’s info)

The policy number
-
A completed LA No Fault Assignment of Benefits Form (provided by our office)
Workers Compensation
Before arriving at Prestige Urgent Care, you must report this injury to your on-duty manager or on-call personnel.
Please be sure you have the following with you:

The name and contact info for the person you reported the injury to.

Your company’s Workers Compensation Carrier (this should be posted in the company breakroom).

An open and active claim number, if it has already been obtained, as well as any adjuster contact information you have been provided.

If provided to you by your employer – a copy of the accident report and completed NYS Workers Comp Form C2.

If your employer states they would like to pay for your visit (instead of going through Workers Compensation).

They must provide a contact number that we can reach at the conclusion of your visit to make a credit card payment over the phone.

If they require an invoice we must have a correct mailing address and/or fax number and name for the responsible party, and documentation that the accident was reported.
